ATO Forms on the Internet

Electronic Forms

Some commonly used ATO forms are available on the Internet for downloading. They can be found on the ATO web site under Electronic Forms. The intelligent electronic forms have been built using JetForm. The user can download the appropriate Form (which includes an in-built expiry date) and the Filler product to operate the "intelligence" of the Form. Special programming capabilities incorporated into the software validate details such as TFNs and accurately calculate the family tax allowance. The user can access his database for information and then save the completed form onto the hard drive and/or print the (partially) completed form from their local printer.

The JetForm forms on the ATOassist site fall into two main categories. Forms which transmit electronically to the ATO (Transmittable forms) and forms which must be filled out, printed and mailed into the ATO (Fill and Print forms). The majority of the forms on the ATOassist site are Fill and Print forms.

Benefits to ATO clients include:

  • improved timely access to current forms;
  • reduced storage and disposal costs for holding ATO forms;
  • validation and error correction available online at the time of keying;
  • on screen completion assistance available as "help files" or "informative messages",
  • transmittable forms save time and postage/delivery,
  • transmittable forms receive confirmations from the ATO

  • Application for variation of amounts required to be withheld under PAYG Income Tax Withholding -1 July 2001 to 30 June 2002 (NAT 2036 – PDF format)
  • Appointment of Tax Agent – Individuals
  • Appointment of Tax Agent – Non-Individuals

  • Notice of Non-lodgment of 2001 Fringe Benefits Tax Return
  • Tax File Number application or enquiry – Individual (NAT 1432)
  • Tax File Number application or enquiry – Individual living outside Australia (NAT 2628)
  • Tax File Number application or enquiry – Non-Individuals. This form is no longer available. *Note: Applicants should now complete the relevant parts of the Australian Business Number (ABN) Registration form
  • Tax File Number Declaration (NAT 3092)

Transmittable Forms

  • Choosing to pay an Annual Pay As You Go (PAYG) Instalment (NAT 3133)
  • Investment Income Report – Amended Annual Investment Income Report (AAIIR)
  • PAYG Annual Income Tax Instalment Variations
  • TFN Declaration (NAT 3092) (pilot) will be replaced by TFN declaration (transmittable) and Withholding declaration (non-transmittable).
  • 2001 Fringe Benefits Tax Return Form (NAT 1067)
